Third Party Claim Administrator, Utilization Management/Pre-Certification/Case Management
AmeriBen Benefit Solutions is responsible for maintaining eligibility records, administering benefit claims, administering Flexible Spending Accounts, coordinating Health Savings Account administration, answering benefit inquiries, administrating COBRA, and handling other routing administrative functions. Also, provides utilization review services, including pre-certification, second and third surgical options and case management services.

Blue Cross Blue Shield - Preferred Provider Network
The pool contracts with the Blue Cross Blue Shield network, which is one of the largest network providers in the State of Arizona. In addition, it is a PPO plan so plan participants may choose any doctor within the network without a referral from a primary care physician.
CVS Caremark - Prescription Benefits
Catamaran is the new pharmacy benefit manager for the Arizona Public Employers Health Pool. CVS Caremark has a network of pharmacies nationwide. All major pharmacy chains that were available through the Informed RX network are also available though the CVS Caremark network.

Delta Dental of Arizona – Dental Provider
Delta Dental of Arizona provides preventative, diagnostic, and restorative dental services.
Vision Service Plan - Voluntary Vision Benefits
The pool provides a voluntary vision benefit plan through the Vision Service Plan (VSP) network. VSP provides coverage for eye exams, lenses and frames.
Health Equity - HSA Administration
A Health Savings Account (HSA) is only when you enroll in the HDHP medical plan option. HSAs are designed to allow individuals to use tax-favored contributions to pay for eligible healthcare expenses.
EAP Preferred – Employee Assistance Program
The pool offers you access to an Employee Assistance Program (EAP). Under the EAP program, both you and your family are offered a variety of services to help identify and address problems that impact your life—all at no additional cost to you.
Reliance Standard Life Insurance Company - Life Insurance
The pool offers a $50,000 life policy for employees and a $10,000 life policy for retirees. There is a $2,000 life benefit for dependents. Plan participants have the option to buy additional limits of coverage on a voluntary basis.
Other - Preferred Provider Network
In addition, the pool has contracted with Mayo Clinic of Arizona.
