Prescription Drug Coverages

All medical options include the prescription drug coverage listed in the 2016-2017 medical benefits chart in your open enrollment guide.

Prescription drug benefits are based on a formulary of preferred drugs. The list below is a guide identifying generic medicines (those should be considered first for prescribing) and preferred brand-name medicines within select therapeutic categories; the list may not include all drugs covered and is subject to change throughout the plan year. The medications would require a copay and are applicable under the Core and Copay plans. If you elected the High Deductible High plan (HDHP) you may utilize the Value Formulary Preferred Drug List, however full-price payment would be required for the medication.

For HDHP participants only: In addition to utilizing the Formulary (costs are at full-price), you may use the Preventive Therapy Drug List below for certain medications, subject to change throughout the year, with a required copayment amount.

Prescription Drug Programs

CVS Caremark is the pharmacy benefit manager for the Arizona Public Employers Health Pool. CVS Caremark has a network of pharmacies nationwide.

How to use the retail program

  • Take your new prescription(s) to a participating retail pharmacy
  • Present your NEW Rx prescription ID card and prescription(s) to the pharmacist. If you have refills left on an existing prescription, you only need to present your new ID card to the pharmacist. The pharmacist will tell you your correct copayment. There are no claim forms to file and no waiting for reimbursement if you fill your prescription(s) at a participating pharmacy.

How to use CVS Caremark by mail

  • If you are currently taking medication on an ongoing basis, consider taking advantage of CVS Caremark by mail.
  • You must get a new prescription from your doctor to take advantage of the CVS Caremark by mail program. Ask your doctor to write a prescription for up to a 90-day supply, plus refills. Mail your prescription and required co-payment along with the order form.

90-Day retail available

  • The plan offers the 90-day retail program, subject to the same copays as mail order.

Dental Benefits

Dental benefits are important to your overall health. The plan offers dental benefits through the Delta Dental of Arizona network. Delta Dental provides preventive, diagnostic, and restorative dental services. Delta Dental network providers accept pre-negotiated fees, allowing you to pay less for the services you receive.

Life Insurance and Accidental Death & Dismemberment (AD&D) Benefit

The pool offers, through Reliance Standard Insurance Company, a $50,000 life policy for employees and a $10,000 life policy for retirees. There is a $2,000 life benefit for dependents. Plan partcipants have the option to buy additional limits of coverage on a voluntary basis.

Voluntary Vision Benefits

The pool also provides voluntary vision benefits. Vision Service Plan (VSP) is the provider network. If you elect this coverage, you can see a VSP provider and receive coverage for eye exams, lenses, and frames.
